Arfinn Med allows users to add additional fields to their intake form to capture more patient information. This can be done by hovering over the settings tab on your Dashboard and selecting the 'Form Builder' option.
Once in the form builder interface, providers can select from different question types and drag and drop them into place. Questions can be edited and made optional or required. We also allow providers to code answers with different values to provide additional insight. Once all questions have been completed, click the save button. The custom form questions will now be viewable at the bottom of the existing intake form.
